Take control, gain confidence to self-manage chronic conditions



 

Lee Health’s mission is to be a trusted partner, empowering healthier lives through care and compassion.

For those living with chronic health conditions, like arthritis, diabetes, heart disease or high blood pressure, having a trusted partner, gaining a level of understanding and being empowered to self-manage their conditions can help them avoid complications and hospitalizations.

A free, six-week program called “It’s All About You” is one example of the ways Lee Health aims to live our mission.

Originally developed at Stanford University Patient Education Research Center, It’s All About You teaches people 18 years of age and older how to better manage their chronic conditions through decision-making, action-planning/goal-setting and problem-solving.

The program stresses the importance of managing signs, symptoms and medications, as well as exercise, healthy nutrition and stress management. Participants are led through the classes by facilitators who also live with chronic conditions, so there is a true understanding by everyone of what it takes to lead a healthy life while managing other health conditions.

In light of COVID-19 and to ensure the safety of all who participate, Lee Health’s Lee Health Solutions pivoted to provide the classes virtually rather than in-person.

The next program, which includes six weeks of 2.5-hour classes, starts Tuesday, Aug. 4, from 1-3:30 p.m. To sign up for the class or for more information, call 239-343-9264.
